Below is a letter and a form for service in Colorado for anyone interested.

Hello friends and pescadores,

This is the form we will use for RMTDW #1, (men's, 9/15-18)(women's, 9/22-25) and probably subsequent weekends. When you print it out, try to make it on one page, front and back if you can. Send it to me, the Levans bwlevan@comcast.net , or John Copper jacopper@tds.net . The team fee is $ 145.00 for #1 weekend, $135.00 for candidates and a $20.00 sponsorship fee. That makes what the candidate pays $115.00. If you are sponsoring multiple candidates the $20 sponsorship fee can be paid by the candidate as that $20 can mount up in multiples and we don't want to not invite candidates over this practice. If there are any who need scholarships, either candidates or team members please let us know. If you want to fill out the team application on line I will forward it to our rectors when you return it. This certainly is the quickest way to get it back to us. The team selection is in progress now so don't delay and if you make the commitment we expect you here unless the Lord returns first, ok? Well, maybe some other things would be considered. Your name has already been provided to the rector so the information on this form is vital to us. Please know the Evil One will try every way to keep this weekend from happening and you from being here, pray for a hedge of protection round us all and especially the candidates.
